How do I schedule a tour?
Fill out the contact form above and select "Schedule a Tour" as your subject, or call (302) 449-3400. Tours are available Monday–Friday, 9 AM – 4 PM, and weekends by appointment.
What services do you offer?
We offer post-acute rehabilitation, skilled nursing, long-term care, memory care, respiratory therapy with ventilator support, and on-site dialysis at select facilities. Visit our Services page for details.
Do you assist with Medicaid applications?
Yes — our social workers guide Delaware families through the DSHP Plus Medicaid application process, including financial eligibility, asset spend-down, and coordination with the Division of Social Services.
What are your visiting policies?
We welcome visitors with flexible hours — typically 8 AM to 8 PM daily with extended hours available. Family involvement is central to our care approach and we strongly encourage regular visits.
How quickly can a loved one move in?
For hospital discharges, we can often complete a clinical review and accept move-in within 24 hours. Our care team coordinates directly with hospital case managers so the transition feels smooth — not scrambled.
